The purpose of this How to Guide is to provide instructions on how to assign surveys to a participant
- Login to the physIQ portal using individual login credentials
- From the Watchlist, select the participant profile by selecting the participants display name
- From the Patient Dashboard, select Patient Settings
- Select Questionnaires
- Select Assign Questionnaires
- The Assign Questionnaire window will appear. Review the fields within the window and modify as needed
a. Tag: Select the appropriate Tag that is associated with the survey. If there is only 1 tag, it will automatically be selected
b. Questionnaire: Select the appropriate Questionnaire (Survey) that needs to be assigned. Questionnaire's will display in alphabetical order
c. Start Date and Time: The date and time of when the survey will be sent to the participants physIQ phone. The current date and time will display as the default. To change the date and time, select the calendar icon
d. Questionnaire Preview: This is the display of the questionnaires that will be sent to the participants physIQ phone
e. More Options: If the questionnaire requires an expiration date, or the questionnaire needs to be completed on a recurring basis, select More Options and proceed to Step 7
f. If no further options need to be set for the Questionnaire, select Send Questionnaire and proceed to Step 10
- By selecting More Options, the Questionnaire Frequency window will appear. Once the Questionnaire Frequency is set, select Continue
a. On Demand: This will allow the surveys to remain available after the participant completes a survey. Select Yes for the surveys to always be available, select No for the surveys not to display on demand for the participant
b. Recurring: This will allow the surveys to become available at a set date and time at a set frequency. Select Yes to set the surveys at a recurring date and time, at a specific frequency. Select No to prevent the survey from recurring
- The Date Questionnaire Expires window will appear. Once the expiration date is set, select Continue
a. Expires: This will allow an expiration date to be set on the assigned survey. Select Yes to set the expiration date. Select No to allow the survey NOT to have an expiration date
- Review the Confirmation screen to ensure all of the requirements of the survey assignment are correct. Once confirmed, select Assign Questionnaire
- The Questionnaire is now sent to the phone based on the date and time set. The user will be returned to the Patient Settings page and a notification will appear on the bottom right corner of the screen indicating the questionnaire was successfully assigned
